This is a carefully documented, multi-year pilot project evaluation concerning the development and implementation of a coordinated service referral system for parents and newborns in eight health and social service centres in Montreal. The origin of the project was an inquest in 1985 into the death of an infant due to parental neglect; it recommended better information sharing among service providers in order to support families during the perinatal period. Il s'agit de l'évaluation soigneusement documentée d'un projet-pilote pluriannuel qui consistait à élaborer et à mettre en œuvre un système coordonné de services d'aiguillage pour les parents et les nouveau-nés, dans huit centres montréalais de soins de santé et de services sociaux. Le programme a son origine dans une enquête de 1985 sur le décès d'un bébé survenu par suite de la négligence des parents; les enquêteurs recommandaient que les pourvoyeurs de services échangent plus d'information pour être en mesure de mieux aider les familles au cours de la période périnatale.
