To provide system-wide planning expertise to the community, health service providers, and the Ontario Ministry of Health. CDHC strives to be an impartial and objective leader in promoting improved population health status and outcomes. CDHC seeks to assist health partners in maximizing their effectiveness, by conducting needs analysis, fostering appropriate service development and linkages, and ensuring accountability. The Champlain District Health Council (CDHC) was established by Order in Council in December 1997. Formed through the amalgamation of three former DHCs: the Renfrew County DHC, the DHC of Eastern Ontario, and the Ottawa-Carleton Regional DHCS it covers the following geographic area: County of Renfrew, United Counties of Prescott and Russell (P&R) United Counties of Stormont, Dundas and Glengarry (SD&G) , City of Ottawa.
