Public Health Agency of Canada. This report is dedicated to the memory of Sir Richard Doll, who had a long and distinguished career as an epidemiologist. Richard Doll died on July 24 2005, at the age of 92. His talk "Progress against cancer: an epidemiologic assessment" at the 1991 Meeting of the Society for Epidemiologic Research in Buffalo, New York, subsequently published in American Journal of Epidemiology (volume 134, 1991) was the inspiration for the work presented in this report.
