If any action level identified as an administrative control level in the MNR [McMaster Nuclear Reactor] Radiation Safety Program document (hereinafter "HP-9000") is reached or exceeded, the licensee shall, immediately after becoming aware of the matter, file a report on the matter to the Commission and shall file a final written report to the Commission in accordance with section 11.3 of HP-9000.
