DEF

An identification [form] used to describe goods exported or destroyed under customs supervision. Each shipment documented on [this document] must be examined by a customs inspector and the inspector must certify that the goods have been exported or destroyed.

OBS

The importer/exporter or his agent is responsible for the completion of [this] form. The form must be submitted in duplicate with the goods to be examined. When packing or crating is necessary, consideration must be given to the fact that customs inspectors are required to identify the goods before exportation or destruction.
