Health Canada, Health Care Policy Directorate. The Chronic and Continuing Care Division provides analytical and policy leadership in relation to primary health care, continuing care (including home care) and palliative/end of life care. As such it is responsible for monitoring the system in these areas, identifying policy issues and providing policy analysis and policy/strategic advice as to federal or federal/provincial/territorial (FPT) implications and action.
