Un choc électrique de haut voltage entraîne une dépolarisation simultanée et synchrone de toutes les fibres cardiaques qui permet une resynchronisation de l'activité électrique et mécanique du cœur à condition que le myocarde soit correctement oxygéné, donc précocement dans la chaîne de survie. Le choc électrique en courant continu est produit par une décharge de capacitance délivrant jusqu'à 400 Watts.s-1 (joules) en 0,01 secondes environ. On utilise généralement une énergie de 3 à 5 joules/Kg.
