Les interprétations peuvent être exogènes (porter sur la réalité extérieure) ou endogènes (porter sur les phénomènes corporels du sujet). Le thème central est presque toujours la persécution, la malveillance, le préjudice. Il peut être parfois partagé par l'entourage du sujet, on parlera alors de «délire induit» ou de «délire à deux».
