En chirurgie classique, le chirurgien ouvre la paroi et crée ainsi un champ opératoire à l'air libre. En chirurgie vidéo-endoscopique, ce champ opératoire est formé par une "bulle d'air" créée en injectant du gaz dans l'espace qui sépare les deux feuillets du péritoine (les deux membranes qui recouvrent l'intestin), ou de la plèvre (les deux membranes qui recouvrent les poumons).
