Dans le cas d'une irradiation très localisée, les tissus les plus exposés sont la peau et les téguments sous-jacents; ce type d'irradiation est fréquent dans les accidents d'irradiation, par exemple quand la source est malencontreusement prise à la main par un employé, voire placée dans une poche. Le pronostic est alors fonctionnel, et dans les cas graves l'évolution peut justifier l'amputation d'un segment de membre irrécupérable.
