Through this program, individuals who are limited in their ability to carry out normal daily activities and require assistance are able to have their support service needs assessed in order to determine if they qualify for any of the services. The assessment is done by a social worker and a nurse along with the person and the family. Together they will determine the types of services that are necessary.
