Il faut différencier la contusion pulmonaire simple isolée de la contusion pulmonaire avec insuffisance respiratoire. La gravité d'une contusion ne sera pas appréciée sur les radios mais sur les gaz du sang, le patient respirant en air ambiant ou en cas de doute en oxygène pur. Seule la répétition des gaz du sang pourra permettre de déceler le passage d'une forme à l'autre au début du traitement.
