The System Navigator provides intake services for new clients and provides a comprehensive assessment of their strengths, capacities and needs across the social and physical determinants of health. They provide clients with assistance in navigating the broader health care system in coordination with the Health Centre’s multi-disciplinary approach. The incumbent will work in collaboration with other Health Centre staff, representatives of the neighbourhood and relevant community agencies within the framework of the Health Centre’s mission and vision of an integrated, multidisciplinary approach.
