Les clous sont essentiellement utilisés pour pratiquer l'ostéosynthèse des fractures des os longs à canal médullaire large (fémur, tibia). La contention est d'autant meilleure que le contact entre le clou et le canal médullaire est plus étroit évitant ainsi les mouvements de rotation. Certains alèsent le canal médullaire pour employer le clou le plus gros possible.

CONT

L'utilisation d'un clou alésé nécessite un temps opératoire plus long et est associée à un taux d'infection inacceptable en présence de fractures ouvertes de classe III.
