À un stade extrême de la spondylarthrite ankylosante (ankylose de la colonne vertébrale), le sujet a la déformation dite en «pantin de bois». Le rachis est infléchi en avant. La lordose lombaire est effacée. Il existe une importante cyphose dorsale et une projection en avant du rachis cervical. Les genoux sont fléchis ainsi que les hanches, comme pour préserver le champ visuel.
