Sur pupille myotique, on réalise une iridectomie sectorielle à partir d'une iridectomie périphérique.

L'opacification du cristallin peut commencer par la périphérie, laissant pour un temps le noyau transparent; le malade est alors plus gêné à la semi-obscurité qu'à la lumière vive, car, dans ce cas, la pupille en myosis fait écran et masque les opacités, supprimant ainsi des phénomènes de diffraction très gênants et responsables souvent d'une diplopie.
