Obstruction of the subclavian artery proximal to the origin of the vertebral artery.

OBS

Blood flow through the vertebral artery is reversed and the subclavian artery thus "steals" cerebral blood, causing symptoms of cerebrovascular insufficiency (subclavian steal syndrome).
