Certaines capsules postérieures fibreuses restent compatibles avec une excellente vision, et inversement certaines capsules assez claires sont responsables de baisses visuelles importantes (perles d'Elschnig). Le traitement repose sur la capsulotomie chirurgicale ou au laser YAG.
