A test for thoracic outlet syndrome : with the patient in a sitting position, his hands resting on thighs, the examiner palpates both radial pulses as the patient rapidly fills his lungs by deep inspiration and, holding his breath, hyperextends his neck and turns his head toward the affected side. If the radial pulse on that side is decidedly or completely obliterated, the result is considered positive.

Forced inspiration is caused by the contraction of muscles in the upper chest, such as the sternocleidomastoids & scalenes. These muscles pull the chest up and out even more, further decreasing the intrapleural pressure so that even more air will rush into the lungs to fill the void.

In a healthy subject everything works together to create large flows during a forced inspiration : 1 The greater the inspiratory force applied, the lower the alveolar pressure relative to pressure at the mouth, and the larger the flow into the lung. 2 During a forced inspiration the(subatmospheric) pleural pressure outside intrathoracic airways is lower than bronchial pressure. The pressure difference is largest at the thoracic outlet and smallest in small intrapulmonary airways. Hence large intrathoracic airways... are subjected to a considerable distending force, and this helps in generating high inspiratory flows.

Any of a variety of neurovascular syndromes resulting from compression of the subclavian artery, the brachial plexus nerve trunks, or less often the axillary vein or subclavian vein, by thoracic outlet abnormalities such as a drooping shoulder girdle, a cervical rib or fibrous band, an abnormal first rib, or occasionally compression of the edge of the scalenus anterior muscle. Continual hyperabduction of the arm may cause another variety(hyperabductions). Arterial compression leads to ischemia, paresthesias, numbness, and weakness of the affected arm, sometimes with Raynaud's phenomenon of the arm. Nerve compression causes atrophy and weakness of the muscles of the hand and, in advanced cases, of the forearm, with pain and sensory disturbances in the arm. Venous obstruction usually takes the form of the Paget-Schroetter syndrome.
