A test used in the examination of patients with cervical spine syndromes. This test should be done with the patient seated; the open palm of the examiner's hand should be placed under the patient's chin while the examiner's other hand goes under the patient's occiput; gradually the force of lifting the patient's head increases, removing the weight of the skull and distracting the foramina, discs, and joints. When performing the straight leg raise test the patient is positioned supine in the absence of pillows. The clinician lifts the patient's symptomatic leg by the posterior ankle while keeping the knee in a fully extended position. The clinician continues to lift the patient's leg by flexing at the hip until pain is illicited or end range is reached. Neurologic pain which is reproduced in the leg and low back between 30-70 degrees of hip flexion is a positive result of lumbar disc herniation at the L4-S1 nerve roots. In order to make this test more specific, the ankle can be dorsiflexed and the cervical spine flexed. Any of a variety of neurovascular syndromes resulting from compression of the subclavian artery, the brachial plexus nerve trunks, or less often the axillary vein or subclavian vein, by thoracic outlet abnormalities such as a drooping shoulder girdle, a cervical rib or fibrous band, an abnormal first rib, or occasionally compression of the edge of the scalenus anterior muscle. Continual hyperabduction of the arm may cause another variety(hyperabductions). Arterial compression leads to ischemia, paresthesias, numbness, and weakness of the affected arm, sometimes with Raynaud's phenomenon of the arm. Nerve compression causes atrophy and weakness of the muscles of the hand and, in advanced cases, of the forearm, with pain and sensory disturbances in the arm.
