Le 1er mai 2003, lors d'opérations de remplissage de la piscine du bâtiment réacteur, une alarme indiquant un niveau d'eau trop faible s'est déclenchée. Durant les opérations, une succession d'erreurs a conduit à la modification du lignage (c'est-à-dire du branchement des tuyauteries et des ouvertures et fermetures de vannes) et à la vidange lente de la piscine.
