Angiotensin II receptor–neprilysin inhibitors. Sacubitril-valsartan is a fixed dose combination of the angiotensin II receptor blocker valsartan and the neprilysin inhibitor sacubitril. ... Sacubitril/valsartan is a combination product. Sacubritil acts as a neprilysin inhibitor and works by blocking the action of neprilysin, inhibiting the breakdown of natriuretic peptides. This results in increased diuresis, vasodilation, reduction in blood pressure and work on the heart. Valsartan is an angiotensin receptor blocker and inhibits the RAAS [renin-angiotensin aldosterone system] ... to reduce the maladaptive response to cardiac failure. Neprilysin breaks down angiotensin II, so sacubitril given alone would result in an accumulation of angiotensin II. Angiotensin II receptor blockers do not inhibit ACE [angiotensin-converting enzymes]. L'angiotensine III [...] a environ 40 % de l'activité hypertensive de l'angiotensine II, mais elle a la même puissance pour stimuler l'aldostérone. Certains chercheurs ont émis l'hypothèse que l'angiotensine III est le peptide naturel qui stimule l'aldostérone, tandis que l'angiotensine II est le peptide qui contrôle la pression sanguine. Cela semble toutefois peu probable, et on croit plutôt que l'angiotensine II est un produit de dégradation qui a conservé une certaine activité biologique. Angiotensin II (Ang II) raises blood pressure (BP) by a number of actions, the most important ones being vasoconstriction, sympathetic nervous stimulation, increased aldosterone biosynthesis and renal actions.
