La carte de note a d'abord été un carton sur lequel on inscrivait la note accordée à l'athlète en sports jugés (gymnastique, plongeon, nage synchronisée, saut à ski, etc.). L'ont remplacée les séquences de cartons en numérotation continue articulés sur un rabat, que les juges utilisaient pour composer la note (avec centièmes de points) attribuée.
