A noninvasive breast tumor that takes the form of proliferation of the ductal epithelium, and which tends to grow within the ducts.

Ductal carcinoma in situ [is] a breast carcinoma characterized by relatively large and-or pleomorphic carcinoma cells that grow, fill, and plug the ducts and the ductules with carcinoma cells, but remained confined within the basement membrane. Various patterns are present: solid, cribriform, and papillary. Ductal carcinoma in situ are of two variants: well differentiated variants and poorly differentiated, pleomorphic, in situ tumors.

Selon qu'ils se développent à partir de l'épithélium des terminaisons ductulaires ou de celui de plus gros canaux, on distingue les carcinomes lobulaires in situ (CLIS) et les carcinomes canalaires in situ (CCIS) ou carcinomes intracanalaires ou carcinomes ductaux in situ. Mais cette distinction n'est pas aussi tranchée, car 75 % des CLIS diffusent dans les canaux adjacents et 45 % des CCIS peuvent envahir les lobules par voie rétrograde. De plus, certains cancers nés au niveau de l'unité termino-ductulaire de Wellings (TDLU) pourraient adopter les deux types en même temps.

Ce carcinome comporte 4 types architecturaux : massif, en comédons, papillaire et cribriforme.

La grande majorité des épithéliomas [ou carcinomes] mammaires sont d'origine glandulaire. Il s'agit d'adénocarcinomes. Les épithéliomas malpighiens mammaires n'existent qu'en très petit nombre. De plus, selon l'extension, on doit d'emblée distinguer deux aspects : l'adénocarcinome infiltrant et l'adénome «in situ». Le cancer infiltrant, quelle que soit sa structure anatomopathologique, présente une tendance extensive vis-à-vis du conjonctif. L'invasion lymphatique et ganglionnaire est prévisible. Dans le cas du cancer «in situ» (ou non infiltrant) la barrière constituée par l'épithélium des galactophores ou des lobules n'est pas franchie.

La très grande majorité des cancers du sein se développe aux dépens de l'épithélium des canaux galactophores ou des lobules glandulaires, réalisant des adénocarcinomes plus ou moins différenciés. Dans cette catégorie on distingue : a) Les carcinomes non infiltrants, carcinomes lobulaires «in situ», carcinomes intracanalaires. b) Les carcinomes infiltrants. c) Les carcinomes particuliers en raison de leur modalité évolutive propre ou de leur morphologie spéciale - les formes médullaires ou «à stroma lymphoïde» bien circonscrites, - les formes colloïdes à composante myxoïde abondante, bien circonscrites aussi - les carcinomes à lobules infiltrants, à cellules isolées, souvent très mal limitées, en squirrhe, diffusant plus loin et se bilatéralisant plus fréquemment que les autres formes de carcinomes.
