Cette opération consiste en : - une excision elliptique transversale de la peau, sur une largeur d'environ 2 à 3 mm, sur la face dorsale de l'articulation I.P.D.; - l'excision du cal tendineux à ce même niveau, allant jusqu'à la capsule qu'on évite d'ouvrir, ce qui n'est pas toujours possible; - quatre fils monofilament de nylon 3-0 sont passés à travers la peau, le tendon, la capsule, d'un côté puis de l'autre; ils ne sont pas immédiatement noués, mais en croisant les fils, on teste que l'extension complète de l'articulation peut être obtenue.
