An ex-vessel steam explosion may occur if the primary system is breached by hot debris. Debris in the reactor outlet piping may fail the piping and fall onto the pump room floor or debris on the reactor vessel bottom may be blocked from flowing into the outlet piping if it freezes in the effluent piping nozzle muff structure. If this occurs, the debris will heat up on the reactor vessel bottom and eventually fail the bottom shield. The likely mode of failure of the bottom shield is collapse of the shield as a whole. If it collapses, the entire shield, as well as the molten debris, will fall into the pin room below the reactor vessel. The debris will then flow across the floor into the pump room. In either event, a large portion of the reactor fuel inventory will be deposited on the pump room floor. The pump room floor will be covered with water in many scenarios. A steam explosion may result when the debris enters this water. 1, record 4, English, - ex%2Dvessel%20explosion

According to the report, the cause of the mishap was the failure of the vacuum line that became disconnected at a "T" fitting. The vacuum line controls the flow of air that mixes with fuel to power the aircraft. The loss of this line stalled the aircraft due to excess air in the fuel upon takeoff and resulted in the crash. 2, record 13, English, - vacuum%20line

Significant temperature gradients are present across the fuel particles when a nuclear reactor is producing power. If local temperatures are sufficiently high, nuclear fuel kernels can migrate up the thermal gradient. During this process, the barrier retaining fission product is progressively damaged, and the damage can lead to complete failure of the coating system. L'interaction pastille-gaine.

Overspeed conditions can occur in an engine for a number of reasons, such as for example, a sudden unforeseeable reduction in the engine load, a failure in the metering valve of the engine's fuel delivery system, or an erroneous signal from an associated electronic engine control(EEC) which modulates the metering valve to an excessively high flow setting. It is known to provide an overspeed protection system that monitors a speed of the engine, such as the rotational speed of the power turbine of a gas turbine engine, and upon detection of an overspeed condition will reduce or stop the flow of fuel to the engine from the fuel delivery system. 2, record 21, English, - overspeed%20protection%20system

The consensus of the U. S. EPA and the automotive industry is that the fuel antiknock additive methylcyclopentadienyl manganese tricarbonyl(MMT) has significant adverse effects on HC emissions. It is the judgement of the authors that it would be very difficult for manufacturers to certify vehicles at the proposed levels unless the HC standard were relaxed to compensate for the predicted HC increases. However, the authors are especially concerned with the potential for premature failure of oxygen sensors if MMT fuels are used. 1, record 24, English, - MMT%20fuel
