Intraocular gas tamponade has become an integral part of macular hole surgery. It is thought to improve success rates of hole closure by providing a tamponade which either forces the healing hole against the retinal pigment epithelium or isolating the hole from liquefied vitreous humor. Various measures have been employed to lengthen the duration of contact between the healing macula and the tamponade agent, including(1) face-down posturing,(2) the use of longer-acting gases and(3) lens extraction to increase the volume of gas used.

Some authors suggest the removal of any glial tissue overlying(or adjacent to) the optic pit. One study looked at nine patients undergoing PPV [pars plana vitrectomy], PVD [posterior vitreous detachment] induction, laser photocoagulation and gas tamponade. Serous maculopathy resolved in six out of six patients in which the glial tissue surrounding the optic pit was removed.... Though glial tissue peeling has proven beneficial in a limited group of studies, precaution must be taken when performing this procedure due to the glial tissue's close proximity to the optic nerve.

Silicone oil was first described as an intraocular tamponade for retinal detachment in 1962... Indications for the use of silicone oil tamponade include complex retinal detachments due to proliferative vitreoretinopathy(PVR), giant retinal tears, traumatic retinal detachments, and some cases of tractional retinal detachments(TRD). There are several advantages to using silicone oil over gas as a tamponade agent. Unlike gas, silicone oil remains in the eye until it is surgically removed and thus may be beneficial for inferior detachments or in patients who are unable to cooperate with postoperative positioning requirements... Additionally, silicone oil volume is unaffected by atmospheric pressure, so it may be used in patients who must undertake high altitude travel.
