To do a gastric bypass, the surgeon makes a small pouch, called a gastric pouch, in the upper part of the stomach. The pouch is about the size of an egg or golf ball. The surgeon also separates the upper part of the small intestine from the middle part of the small intestine. The middle part of the small intestine is then attached to the gastric pouch. This directs food into the middle part of the small intestine, instead of the lower part of the stomach.

Restrictive gastric operations, such as vertical banded gastroplasty(VGB), serve only to restrict and decrease food intake and do not interfere with the normal digestive process. In this procedure the upper stomach near the esophagus is stapled vertically to create a small pouch along the inner curve of the stomach. The outlet from the pouch to the rest of the stomach is restricted by a band made of special material. The band delays the emptying of food from the pouch, causing a feeling of fullness.

Biliopancreatic diversion is primarily a malabsorptive procedure. The standard procedure involves the removal of part of the stomach(a limited horizontal gastrectomy) to limit oral intake and induce weight loss. The gastric pouch that is created is larger than that of gastric bypass or the restrictive procedures, therefore allowing larger meals, and patients remain on a less restricted diet than would the case following gastric bypass. Part of the small intestine is also bypassed by the construction of a long limb Roux-en-Y anastomosis with a short common "alimentary" channel of 50 cm length.

Gastric bypass is one common type of weight-loss surgery. During bypass surgery, the top of the stomach is changed into a small gastric pouch. A loop of your small intestine is cut, and one end of the loop is brought up and is connected to the gastric pouch. This connection is one anastomosis. The other end of the small intestine loop is reconnected to the small intestine, further down. This is another anastomosis. Food is then redirected to an area farther down in your digestive system. It bypasses the stomach. Because food will now bypass your stomach, your body does not absorb as many calories.
