One of the four autoantibodies often detected in certain thyroid diseases, primarily Hashimoto's thyroiditis, but also in Graves disease and primarily hypothyroidism. It is interpreted as evidence for an auto-immune pathogenesis in these disorders.

These antibodies work against thyroid peroxidase, an enzyme that plays a part in the T4-to-T3 conversion and synthesis process. TPO antibodies can be evidence of tissue destruction, such as Hashimoto's disease, less commonly, in other forms of thyroiditis such as post-partum thyroiditis.

Thyroiditis experimentally induced in laboratory animals following injection of thyroglobulin or thyroid extract in complete Freund's adjuvant. Histopathologically, it resembles Hashimoto's disease.
