Breast-conserving surgery removes the cancer and surrounding tissue. The goal is to take just enough tissue so that the breast looks as normal as possible after the surgery but the chance of the cancer coming back is low. The size and location of tumors differs from one person to another, so the amount of tissue removed during surgery also varies. For breast remodeling, the plastic surgeon uses techniques similar to those used in the breast reduction surgery performed for women who experience pain or discomfort due to large breast size. This approach can yield excellent results, particularly if the tumor was located in the lower half of the breast, and is usually preferred if sufficient breast tissue remains after the partial mastectomy. Choosing an external breast prosthesis after a mastectomy is one option for women who either elect to wait to have breast reconstruction or who choose not to have the additional surgery. An external breast prosthesis is an artificial breast form that can be worn after a mastectomy to help balance the body and anchor the bra on the side of the mastectomy from riding up, which helps prevent back and neck pain and a sagging shoulder. Breast prostheses are custom-made for most women and are made from several different types of material(such as silicone gel, foam, and fiberfill) that feel similar to natural breast tissue and are usually weighted so they feel the same as the remaining breast. During gluteal flap breast reconstruction, a rounded section of skin and fat (and occasionally muscle) is removed either from the top of the buttock or at the crease along the underside of the buttock. After being transferred to the breast, the tissue is shaped to create the breast mound and is attached to the remaining breast tissue. A tissue expander is used so that an implant can fit in place and have a natural shape. It is put under the skin and chest muscles where the breast used to be. Salt water is injected into the expander every two to three weeks for about three to four months. This is done until the chest skin has stretched to the desired size. Surgical sex reassignment. Genital surgical sex reassignment refers to surgery of the genitalia and/or breasts performed for the purpose of altering the morphology in order to approximate the physical appearance of the genetically-other sex in persons diagnosed as gender dysphoric.
