Breast-conserving surgery removes the cancer and surrounding tissue. The goal is to take just enough tissue so that the breast looks as normal as possible after the surgery but the chance of the cancer coming back is low. The size and location of tumors differs from one person to another, so the amount of tissue removed during surgery also varies. To make it simple, you can think of two general breast-conserving surgeries : a lumpectomy and a partial mastectomy.

For breast remodeling, the plastic surgeon uses techniques similar to those used in the breast reduction surgery performed for women who experience pain or discomfort due to large breast size. This approach can yield excellent results, particularly if the tumor was located in the lower half of the breast, and is usually preferred if sufficient breast tissue remains after the partial mastectomy. To achieve symmetry, breast remodeling is usually combined with a breast reduction on the opposite side. The two surgeries are performed at the same time.

An artificial breast form worn in a bra to simulate the shape and weight of a natural breast.

Choosing an external breast prosthesis after a mastectomy is one option for women who either elect to wait to have breast reconstruction or who choose not to have the additional surgery. An external breast prosthesis is an artificial breast form that can be worn after a mastectomy to help balance the body and anchor the bra on the side of the mastectomy from riding up, which helps prevent back and neck pain and a sagging shoulder. Breast prostheses are custom-made for most women and are made from several different types of material(such as silicone gel, foam, and fiberfill) that feel similar to natural breast tissue and are usually weighted so they feel the same as the remaining breast. Some prostheses adhere directly to the chest area and others fit into pockets of postmastectomy bras, which are fitted with pockets on the inside to hold the device in place.

During gluteal flap breast reconstruction, a rounded section of skin and fat (and occasionally muscle) is removed either from the top of the buttock or at the crease along the underside of the buttock. After being transferred to the breast, the tissue is shaped to create the breast mound and is attached to the remaining breast tissue. Using complex microsurgical techniques, the blood vessels are reconnected to existing vessels in the area.

There are two forms of gluteal flap breast reconstruction available after mastectomy : the S-GAP and the I-GAP. S-GAP is an acronym for Superior Gluteal Artery Perforator ;S-GAP reconstruction uses tissue from the top of the buttock, which contains the upper gluteal artery. I-GAP stands for Inferior Gluteal Artery Perforator ;this method uses tissue taken from the crease of the buttock where the thigh and buttock meet and which contains the lower gluteal artery.

An enlarging balloon that stretches the skin after mastectomy.

A tissue expander is used so that an implant can fit in place and have a natural shape. It is put under the skin and chest muscles where the breast used to be. Salt water is injected into the expander every two to three weeks for about three to four months. This is done until the chest skin has stretched to the desired size. The expander is usually removed and replaced by an implant. Some expanders can be kept in place like an implant.
