Il est situé en arrière du noyau moteur du III de part et d'autre de la ligne médiane, en avant de l'aqueduc de Sylvius dans la partie haute du mésencéphale. Les fibres parasympathiques de ce noyau vont vers le ganglion ciliaire, où elles font relais; de là les fibres postganglionnaires se dirigent vers les muscles ciliaires et le sphincter de la pupille. Une interruption de ces fibres entraîne une mydriase.
