Omphalocele(exomphalos) is a developmental defect of the anterior abdominal wall in which the abdominal musculature, fascia, and skin are absent in the midline at the point of insertion of the umbilical cord. Abdominal organs extrude anteriorly through the defect and are covered by a saclike membrane consisting of amnion externally and parietal peritoneum internally. Omphalocele results from failure of the intestine to return to the body cavity after its normal herniation into the umbilical stalk during embryonic life. Omphaloceles vary in size; the defect may be a few centimeters in diameter, or most of the anterior abdominal wall may be lacking. Depending on the size of the defect, small intestine, liver, spleen, and pancreas may be in the sac.

The intestinal limb that is connected to the gastric remnant and serves as the recipient for biliary and pancreatic secretions.

The biliopancreatic limb, also known as the afferent limb, consists of the duodenum and proximal jejunum that remains in continuity with the remnant stomach proximally. The limb contains digestive enzymes from the stomach, hepatobiliary tract, and pancreas. In a standard gastric bypass, approximately 40 cm is measured starting at the ligament of Treitz and divided using a stapling device to create the biliopancreatic(BP) limb.

Insulin resistance typically develops with obesity and heralds the onset of type 2 diabetes. It is as if insulin is "knocking" on the door of muscle. The pancreas makes more insulin, which increases insulin levels in the blood and causes a louder "knock. "Eventually, the pancreas produces far more insulin than normal and the muscles continue to be resistant to the knock. Once the pancreas is no longer able to keep up, blood glucose starts to rise, initially after meals, eventually even in the fasting state. Type 2 diabetes is now overt.
