The first major process in rebuilding a piano is disassembly, starting with the cabinet, then the strings, tuning pins, and finally the heavy cast-iron plate.... In a vertical [piano], first remove the front panels, upper pillars, lid, music shelf, fallboard, nameboard strip, keyslip, keyblocks, action, and keys. If you have a piano tilter..., move the piano far enough from the wall to tip it on its back. Stand the tilter up, center it behind the piano, and push it snugly against the back of the piano with the prongs under the bottom. With one assistant positioned in front to make sure the bottom can’t kick forward, tip the piano and tilter back together. Once the entire weight of the piano is on the tilter, your assistant can stop pushing the bottom of the piano toward the tilter.

This facilitated rapid playing of repeated notes, and this musical device was pioneered by Liszt. When the invention became public, as revised by Henri Herz, the double escapement action gradually became standard in grand pianos, and is still incorporated into all grand pianos currently produced. … In grand pianos, the frame and strings are horizontal, with the strings extending away from the keyboard. The action lies beneath the strings, and uses gravity as its means of return to a state of rest.

A specific type of tenosynovitis which involves the tendons of two specific muscles on the thumb side of the hand - the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). These muscles are located in the forearm and their tendons travel across the wrist (through a "tunnel" termed the first dorsal compartment) on the thumb side to produce movement of the thumb. The walls of the affected tendon sheaths (basically protective coverings for the tendons) become thickened and inflamed.

The precise cause of de Quervain's tenosynovitis is unknown. It is thought that excessive friction from overuse of the thumb and wrist(excessive and repetitive gripping and grasping actions) may be a factor, leading to thickening of the tissues in the compartment housing the APL and EPB tendons. For example, cases have been described in bricklayers, those who sew, piano players, fly fishermen, and golfers. It has also been hypothesized that the condition in some cases may be associated with acute injury to the involved area(direct blow to the forearm or wrist, falling on the thumb).

blackberry thumb: Experts caution that practical experience and emerging anecdotal evidence suggest overuse of personal digital assistants (PDAs) ... can conceivably lead to injury of the tendons and accompanying tissue of the thumb. "Blackberry thumb" is the latest colloquial interpretation of a very real medical condition called de Quervain’s tendonitis, and its variant de Quervain’s tenosynovitis.
