Enzyme found in the urine of mammals, including man, and of other vertebrates; it is elaborated by the parenchymal cells of the human kidney and functions as a plasminogen activator. It is used as a thrombolytic(fibrinolytic) agent.

Plasmin is derived from plasminogen, an inert protein precursor that can be activated by tissue factors (called lysokinases) or by contact through factor XII. An activator, urokinase, is also released by blood-vessel lining and is present in normal urine. Certain bacteria produce plasminogen activators, the streptococci being the most potent. Plasmin is rapidly destroyed in the bloodstream and normally acts only when taken up by ribrin.

Un groupe de 43 malades ayant subi une substitution valvulaire sous CEC a servi à l'établissement des modifications enzymatiques normales après chirurgie cardiaque; une différence est trouvée entre les cas sans complications myocardiques évidentes et 2 cas avec altérations de QRS indiquant l'infarctus myocardique transpariétal postopératoire. Un autre groupe de 63 malades opérés de dérivation aorto-coronarienne comporte 6 cas d'infarctus postopératoire, avec élévation enzymatique marquée, et 46 cas sans signes d'infarctus ni élévation enzymatique. Dans 11 autres cas des élévations enzymatiques sans signes évidents de lésion myocardique sont discutés.

OBS

Parmi les causes d'élévation enzymatique en l'absence d'infarctus, il faut rappeler encore les injections intramusculaires (cause fréquente d'erreur, mais n'entraînant pas d'élévation de la CPK-MB et, surtout, les atteintes cardiaques sans nécrose : péricardite ou myocardite.
