The Baseline (BDI) and Transition (TDI) Dyspnea Indexes provide interview-based measurements of breathlessness related to activities of daily living. The BDI is a discriminative instrument that includes specific criteria for each of three components at a single point in time. The TDI is an evaluative instrument that includes specific criteria for each of three components to measure changes from a baseline state. Observational studies have shown that patients with COPD [chronic obstructive pulmonary disease] generally experience a gradual progression of breathing difficulty as measured by the TDI over time.

Gaensler reste attaché depuis plus de 20 ans à une technique simple qui consiste à faire marcher le sujet pendant 4 minutes à une vitesse lente (3 km 2 par heure) et à mesurer la ventilation pendant les 2 dernières minutes de cet exercice de faible intensité. Il établit ainsi un index de dyspnée qui n'est autre que la ventilation de marche exprimée en pourcentage de la ventilation maximale par minute obtenue de façon indirecte à partir du VEMS.
