Androgen-binding protein(ABP) is synthesized by the Sertoli cell in the testis, where ~80% is secreted into the luminal fluid and the other ~20% is secreted into the interstitial compartment and taken up into the systemic circulation. ABP binds testosterone and transports it in the seminiferous tubule fluid to the epididymis, where it is taken up by a receptor-mediated process into the principal cells of the initial segment and caput epididymis. Once released from ABP, the testosterone is converted to [dihydrotestosterone](DHT) by the epithelial 5α-reductase. ABP is very similar in structure to sex hormone binding globulin(SHBG), which is synthesized in the liver and binds testosterone in the peripheral blood. Le diagnostic de carcinome sera donc posé pour toute lésion papillaire présentant un degré quelconque d'indifférenciation même en l'absence de tout signe d'envahissement et la classification est fonction du degré histologique et du mode de croissance. Le carcinome à cellules transitionnelles degré 1 (G1) se présente sous 2 formes : - carcinome papillaire non invasif degré 1 : les franges épithéliales sont peu épaissies, les anomalies cytologiques et architecturales sont légères; - carcinome transitionnel invasif degré 1 : la forme invasive du degré 1 est toujours papillaire; elle est très rare et l'invasion se cantonne généralement à la sous-muqueuse. Le carcinome à cellules transitionnelles degré 2 (G2) : se présente sous deux formes : - carcinome papillaire non invasif degré 2 : les papilles sont plus courtes, à tendance coalescente. Il existe une certaine désorganisation architecturale, des atypies plus prononcées; - carcinome à cellules transitionnelles invasif degré 2 : la forme invasive du degré 2 est soit papillaire soit exhophytique. Elle est plus fréquente et l'infiltration peut être plus profonde. Le carcinome à cellules transitionnelles degré 3 (G3) se présente sous 3 formes : - carcinome papillaire non invasif degré 3 : les signes de désorganisation architecturale et les atypies sont majeures; - carcinome à cellules transitionnelles in situ : l'aspect est identique à celui de la forme papillaire mais en muqueuse plane. Ces deux formes peuvent s'associer; - carcinome à cellules transitionnelles invasif degré 3; l'invasion est fréquente, souvent profonde survenant sur tous les modes de croissance (papillaire, in situ, ou de novo). Introduction Tenascin is a large hexameric extracellular matrix molecule with oncofetal properties. During embryonic development the protein is expressed in many parenchymatous organs, where it appears to have a regulating role in epithelial differentiation. In most mature tissues the expression is restricted to the papillary dermis, but reappears in tissue regeneration and repair and in neoplasia.
