A deformity of the foot in which the heel is elevated and turned inward from the midline of the leg and the foot is plantar flexed. This is associated with the raising of the inner border of the foot and displacement of the higher part of the foot so that it lies medially to the vertical axis of the leg. With this type of foot the arch is higher and the foot is in equinus. This is a typical clubfoot.

Abnormalities occurring in 10-50 % of cases: Hands and feet: Ulnar or radial deviation of hand. Hypoplastic or absent thumb, simian crease. Equinovarus, rocker-bottom feet. Syndactyly of second and third toes.

Idiopathic flatfoot. There is no deformity in the non-weight-bearing state. On weight bearing, the foot is flat but the heel remains on the ground. The foot is flexible, and the deformity can be passively reduced in infancy and childhood, becoming rigid in later years.... In the standing position, the talus displaces toward the sole but never becomes perpendicular, and the talonavicular displacement is never complete.... When the heel cord is tight, the calcaneus may be horizontal but is never in equinus.

In summary, factors to consider in treatment are(1) the patient's age;(2) the flexibility of the condition;(3) the severity of deformity;(4) the presence of equinus;(5) abnormal shoe wear; and(6) the symptoms. When a child presents with severe flexible flatfoot one should rule out an underlying neuromuscular disorder and perform a complete biomechanical evaluation to ascertain any rotational or angular conditions of the legs that might influence the treatment. If a child is in... group III(ages 13 to 17) adolescent usually has the additional problem of a forefoot varus, which must be controlled using a forefoot post. It is also not unusual to have to add additional wedging inside the heel of the shoe to invert the heel.
