A multilumen tube used for the tamponade of bleeding esophageal varices.

OBS

One lumen leads to a balloon which is inflated in the stomach, to retain the instrument in place, and to compress the vessels around the cardia; another leads to a long narrow balloon by which pressure is exerted against the varices in the wall of the esophagus; and a third provides for aspirating contents of the stomach.

La sonde de Blakemore comporte 2 ballonnets et est introduite par le nez. Lorsqu'elle est descendue dans l'estomac, le médecin injecte 60 cc d'air dans le ballonnet inférieur et retire la sonde afin de bloquer le ballonnet gonflé au niveau de l'extrémité supérieure de l'estomac. Puis, il introduit dans le ballonnet supérieur qui se trouve donc dans l'œsophage 60 à 80 ml d'air qui vont créer une pression de 50 mm Hg. Ce ballon exerce donc une compression directe des varices [œsophagiennes] et permet de stopper l'hémorragie.

Today, advances in medical technology enable speech production by means of a voice prosthesis. By creating a small surgical passage(TEP, or Tracheoesophageal Puncture), inside the stoma, from the back wall of the trachea into the esophageal wall, a small one-inched valved tube(voice prosthesis) can be placed into this passage to enable tracheoesophageal speech. Voice is produced by blocking the stoma, either with a finger or an adjustable tracheostoma valve, so that exhaled air from the lungs can be directed from the trachea through the prosthesis into the esophagus(where vibrations are produced) and then out through the mouth. Fluent, conversational speech is usually acquired within a few days.

La prochaine variante est une prothèse vocale artificielle. Celle-ci est faite d'une valve en silicone qui est mise dans une liaison entre l'œsophage et la trachée-artère, et ceci au niveau de l'ouverture de la trachée-artère au cou par laquelle les patients respirent après l'ablation du larynx. Afin d'utiliser la prothèse vocale, le patient ferme cette ouverture, appelée trachéotomie, l'air souffle sur la valve qui vibre et produit des sons. Les difficultés surgissent au moment où cette valve perd son étanchéité ou que le patient avale en permanence de travers, de sorte à ce que la nourriture arrive de l'œsophage dans la trachée-artère ce qui, dans les pires cas, peut provoquer des pneumonies.

A tube inserted into the esophagus to maintain airway patency in unconscious persons for positive-pressure ventilation through the attached face mask.

OBS

An esophageal obturator airway [or] EOA is a semi-flexible large-bore tube approximately 30 cm in length, with 19 holes in the shaft and an inflatable cuff. A soft face mask is attached to one end and the other end is closed. The airway was designed for personnel who are not authorized to place endotracheal tubes. One of the distinct advantages is that it can be inserted blindly through the mouth without having to visualize the larynx. It is also helpful in the prevention of gastric regurgitation. The disadvantages are that the tracheo-bronchial tree cannot be adequately suctioned and there is the possibility of esophageal rupture when the cuff is inflated too fully.

Surgical intervention to prevent esophageal reflux is indicated for the patient in whom recurrent pneumonia can be reasonably attributed to this mechanism... Bypassing the mouth to facilitate feeding can be accomplished with a feeding gastrostomy or enterostomy. The former can be performed noninvasively via fiberoptic gastroscopy with the feeding tube being passed percutaneously into the stomach.
