One method used to uncover all the things that could go wrong is the "event tree/fault tree" analysis technique. Event tree work is not unlike following a multi-branched trail of dominoes as they knock each other over in sequence. Such analysis allows the tracing of things that could occur after the initiating events. Paths of action and consequences are traced, sometimes to great length. The fault tree technique, a favorite tool of AECB staff members, is a means of finding out what combinations of component and system failures would cause an event in the first place.
