Care maps outline treatment for patients who may require services from several different organizations or providers. By integrating the practices and experiences of each provider, care maps help the physician and the health care team set out a plan for all necessary patient care, thus helping define how each provider contributes to the overall patient plan. This approach improves efficiency by ensuring that all steps are performed at the optimum time... care maps also provide the most timely, efficient and effective access to treatment... care maps help physicians and others identify both trends in treating patients with the same conditions as well as deviations from the trends... Care maps also improve communication between patients and physicians or other health care providers. The patient better understand what role each health care provider plays and the physician is able to educate patients and their families about what to expect and how to speed recovery. 2, record 20, English, - care%20map

In a physician's office, the circle of care would include : the physician, the nurse, a specialist or other health care provider referred by the physician, and any other health care professional selected by the patient, such as a registered dietitian, pharmacist or physiotherapist. In a hospital, the circle of care would include : the attending physician and the health care team(e. g., residents, nurses, registered dietitians, technicians, clinical clerks and employees assigned to the patient) who have direct responsibilities of providing care to the individual. 2, record 22, English, - circle%20of%20care

Officially designated registered nurse(extended class), or RN(EC), NPs [nurse practitioners] are registered nurses with additional education who work with individuals, families, groups and communities. Like all registered nurses, their focus on health promotion and disease prevention facilitates comprehensive health-care reform. Key members of any health-care team, nurse practitioners work in diverse health-care settings, from community health centres to doctors’ offices to hospitals, nursing homes and home care. They diagnose, prescribe and administer medications, order medical tests, conduct Pap smears, educate, and refer patients to other health-care professionals such as nutritionists and social workers. 3, record 26, English, - registered%20nurse%20extended%20class

4, record 26, French, - infirmi%C3%A8re%20autoris%C3%A9e%20%28cat%C3%A9gorie%20sp%C3%A9cialis%C3%A9e%29

The Review of Veterans’ Care Needs is an important policy and research body, established in 1996, that supports the Department's efforts to design programs and services that are driven by client need rather than program requirements, improving our ability to make evidence-based decisions. Through a series of research tools, including a literature review, focus groups with clients, their care givers and a number of health providers, and analysis of health services offered by the community and other levels of government in each province, our Review of Veterans’ Care Needs project team has been able to identify changes in the needs of aging veterans, gaps in benefits and services and VAC can best add value to their care. 1, record 27, English, - Review%20of%20Veterans%26rsquo%3B%20Care%20Needs
