The theory of planned behaviour(TPB) started as the theory of reasoned action in 1980 to predict an individual' s intention to engage in a behaviour at a specific time and place. The theory was intended to explain all behaviours over which people have the ability to exert self-control. The key component to this model is behavioural intent; behavioural intentions are influenced by the attitude about the likelihood that the behaviour will have the expected outcome and the subjective evaluation of the risks and benefits of that outcome. The TPB has been used successfully to predict and explain a wide range of health behaviours and intentions including smoking, drinking, health services utilization, breastfeeding, and substance use, among others.

Of the McGill University-Université de Montréal Research Group on Integrated Services for Older Persons. SIPA Research Program(System of Integrated Services for the Frail Elderly). SIPA is a demonstration and evaluation project based on a model of integrated care for the frail elderly. The demonstration project aimed to implement a model for the frail elderly living in the community and to evaluate by means of an experimental device, its comparative advantage in regards to existing services in Montreal. The evaluation was on impact on health, utilization of services, cost of services, quality of care and the implementation and organization of services. The SIPA project was carried out on the Island of Montreal over a period of 22 months from 1999 to 2001. The SIPA s goals were to : respond appropriately to the needs of older persons with disabilities; maintain and promote the independence and capacity of older persons to make choices, while respecting their dignity; and optimize the use of community, hospital and institutional resources.

De SOLIDAGE (Groupe de recherche Université de Montréal - Université McGill sur les services intégrés pour les personnes ägées. Programme de recherche SIPA (Système de services intégrés pour personnes âgées fragiles). Le projet SIPA est un projet de démonstration et d'évaluation fondé sur le modèle de services intégrés pour personnes âgées fragiles. Le projet de démonstration visait à implanter le modèle auprès de personnes âgées fragiles vivant dans la communauté et à évaluer au moyen d'un devis expérimental son avantage comparatif en regard des services habituels disponibles à Montréal. L'évaluation portait sur l'impact sur la santé, l'utilisation des services, les coûts des services, la qualité des soins, l'implantation et l'organisation des services. Le projet SIPA a été réalisé sur l'île de Montréal sur une période de 22 mois de 1999 à 2001. Les objectifs du SIPA étaient de : répondre à l ensemble des besoins des personnes âgées atteintes d'incapacités; de maintenir et promouvoir l'autonomie des personnes âgées et leur capacité à faire des choix, tout en respectant leur dignité; et d'optimiser l'utilisation des ressources communautaires, hospitalières et institutionnelles.

Preferred Provider Organizations(PPOs) offer purchasers of care several benefits, including expenditure reduction, utilization control, improved quality of care, and efficient management. Although Medicare could benefit from these outcomes, the program lags behind the private sector in PPO development. The Health Care Financing Administration(HCFA) must address several policy issues and constraints as it develops PPOs for Medicare beneficiaries. The agency must identify services and providers to include in the PPO, develop program sponsorship and administration methods, create viable provider and beneficiary incentives to participate, identify sources of PPO cost savings, and examine the role of medigap insurance policies in PPO development.
