A test increasing intrathecal pressure which may produce pain in the cervical spine or back if an intraspinal tumor or a herniated disc is present. The pain may radiate in a dermatome distribution. To perform the Vasalva test, the patient is asked to hold his breath and bear down as if he were moving his bowels.

A maneuver used to detect intrathecal or extrathecal pathology(herniated disc), or pathologic pressure on the theca(wrapping of the cord) : the patient, lying supine on the examining table, raises his extended legs to a position about two inches from the table. This maneuver stretches the iliopsoas muscle and anterior abdominal muscles, and increases intrathecal pressure. The test is positive if the patient cannot hold the position, cannot lift his legs at all, or experiences pain as he attempts the maneuver.

This is most conspicuous in subligamentous disc herniation, where the disc tissue typically extends upward and looks like a hockey stick on sagittal images, while a herniated disc extruding both cranially and caudally looks like a mushroom.

Laser surgery for a herniated disc [where] a thin hollow needle... is inserted into the nucleus of the disc. A strand of optical fibre is inserted through the needle. Then a few bursts of laser energy are sent through and a tiny bit of disc material-which is mostly water-is vaporized. This causes the pressure within the nucleus to drop and a vacuum is created.... The herniated portion of the disc is sucked away from the nerve it is pinching... but only by a fraction of a millimeter.
