Omphalocele(exomphalos) is a developmental defect of the anterior abdominal wall in which the abdominal musculature, fascia, and skin are absent in the midline at the point of insertion of the umbilical cord. Abdominal organs extrude anteriorly through the defect and are covered by a saclike membrane consisting of amnion externally and parietal peritoneum internally. Omphalocele results from failure of the intestine to return to the body cavity after its normal herniation into the umbilical stalk during embryonic life. Omphaloceles vary in size; the defect may be a few centimeters in diameter, or most of the anterior abdominal wall may be lacking. Second impact syndrome can result from even a very mild concussion that occurs days or weeks after the initial concussion. What is Second Impact Syndrome? In patients who sustain a second concussion when the first one has not fully healed, the brain loses its ability to auto regulate intracranial and cerebral perfusion pressure. This may lead to cerebral edema(severe swelling of the brain) and possible brain herniation. Loss of consciousness after the initial injury followed by secondary brain damage creates ionic fluxes, acute metabolic changes, and cerebral blood flow alterations. When performing the straight leg raise test the patient is positioned supine in the absence of pillows. The clinician lifts the patient's symptomatic leg by the posterior ankle while keeping the knee in a fully extended position. The clinician continues to lift the patient's leg by flexing at the hip until pain is illicited or end range is reached.
