Better planning for hospital discharge--Two newly created positions... are improving the rate of patient transfer... and reducing the length of stay for patients... A Discharge Planning Nurse and Residential Care Officer are working closely with some wards... and patients and their families... to ensure patients are safely discharged to their home or a residential care facility sooner.... discharge planning [commences] as soon as patients are admitted... to identify the future needs of patients. This can include social work, physiotherapy, occupational therapy, dietetics, speech pathology or transport needs. These positions create central roles for discharge planning, which improves communication between hospitals, staff and residential care facilities... Patients and their families are now more involved in the discharge process and are able to discuss any issues that need to be considered before a patient returns home or to a residential care facility.

Officially designated registered nurse(extended class), or RN(EC), NPs [nurse practitioners] are registered nurses with additional education who work with individuals, families, groups and communities. Like all registered nurses, their focus on health promotion and disease prevention facilitates comprehensive health-care reform. Key members of any health-care team, nurse practitioners work in diverse health-care settings, from community health centres to doctors’ offices to hospitals, nursing homes and home care. They diagnose, prescribe and administer medications, order medical tests, conduct Pap smears, educate, and refer patients to other health-care professionals such as nutritionists and social workers.
