Les kystes mucineux bénins sont généralement volumineux et multiloculaires et contiennent un liquide mucoïde. Le revêtement cellulaire de type endocervical mucosécrétant forme parfois de petites papilles intrakystiques. Les tumeurs à malignité atténuée sont multiloculaires, avec des zones solides, papillaires, hémorragiques ou nécrotiques. Elles sont difficiles à différencier des adénocarcinomes en ce sens que les tumeurs mucineuses peuvent présenter des aspects histologiques variés, ce qui nécessite des prélèvements multiples pour éviter de manquer des zones d'invasion franche. L'examen microscopique montre un revêtement mucineux stratifié atypique avec des formations papillaires sans invasion stromale (la présence de mucine dans le stroma, ou pseudomyxome ovarien, n'est pas un signe d'invasion).
