Removal of the gallbladder by laparoscopy.

CONT

The laparoscopic cholecystectomy involves the insertion of a long narrow cylindrical tube with a camera on the end, through an approximately 1 cm incision in the abdomen, which allows visualization of the internal organs and projection of this image onto a video monitor. Three smaller incisions allow for insertion of other instruments to perform the surgical procedure. A laser may be used for the incision and cautery(burning unwanted tissue to stop bleeding), in which case the procedure may be called laser laparoscopic cholecystectomy.

La cholécystectomie par laparoscopie consiste à pratiquer quatre petites incisions de 1 cm (1/2 po) sur l'abdomen, dont une au niveau du nombril, afin de permettre au médecin d'introduire les instruments nécessaires pour l'opération. Le chirurgien y insère le laparoscope, c'est-à-dire une lentille et des lumières puissantes reliées à une caméra. Cette caméra est elle-même reliée à un téléviseur qui permet au chirurgien de visualiser l'intérieur de l'abdomen sur l'écran. Il gonfle l'abdomen avec du gaz carbonique afin de bien voir la vésicule biliaire à l'intérieur. Il peut par la suite procéder à l'intervention avec d'autres instruments qui sont insérés dans les orifices. Lorsque l'intervention est terminée, le chirurgien laisse échapper le gaz et retire les instruments.

Du Pont and Duke University collaborators have developed the Biolistic gene gun, a particle delivery system which employs microcarriers coated with genetic material to insert DNA into animal cells. The microcarriers are accelerated on macrocarriers in a stream of helium to fire the microcarriers into external tissue or, using surgery, into the livers of mice. The system has been used to insert genes encoding the enzyme, firefly luciferase, into mice and the next stage is likely to be insertion of a therapeutically useful gene into animal liver cells which would show the procedure to be of potential use in gene therapy.

A chest tube insertion involves the surgical placement of a hollow, flexible drainage tube into the chest.

Chest tube insertions are usually performed as an emergency procedure. Chest tubes are used to treat conditions that can cause the lung to collapse, which occurs because blood or air in the pleural space can hamper the ability of a patient to breath. There are four common conditions that can require surgical chest tube insertion, including : pneumothorax(air leak from the lung into the chest) ;hemothorax(bleeding into the chest) ;empyema(lung abscess or pus in the chest) ;pneumothorax or hemothorax after surgery or from trauma to the chest.

Chest tube thoracostomy is done to drain fluid, blood, or air from the space around the lungs. Some diseases, such as pneumonia and cancer, can cause an excess amount of fluid or blood to build up in the space around the lungs (called a pleural effusion). Also, some severe injuries of the chest wall can cause bleeding around the lungs. Sometimes, the lung can be accidentally punctured allowing air to gather outside the lung, causing its collapse (called a pneumothorax). Chest tube thoracostomy (commonly referred to as "putting in a chest tube") involves placing a hollow plastic tube between the ribs and into the chest to drain fluid or air from around the lungs. The tube is often hooked up to a suction machine to help with drainage. The tube remains in the chest until all or most of the air or fluid has drained out, usually a few days.
