The term cutaneous T-cell lymphoma (CTCL) encompasses a broad group of lymphomatous neoplasms of helper T cells that present in the skin but later may involve lymph nodes, peripheral blood cells, and the viscera. They vary in histology, immunophenotype, and prognosis.

Cutaneous T-cell lymphoma can cause rash-like skin redness, slightly raised or scaly round patches on the skin, and, sometimes, skin tumors. Several types of cutaneous T-cell lymphoma exist. The most common type is mycosis fungoides. Sezary syndrome is a less common type that causes skin redness over the entire body.

OBS

Cutaneous T-cell lymphoma is one of several types of lymphoma collectively called non-Hodgkin’s lymphoma.

Mycosis fungoides(MF) is the most common subtype of cutaneous T-cell lymphoma(CTCL). It is an indolent(slow growing) non-Hodgkin lymphoma(NHL) that develops over many years or even decades. It accounts for around half of all skin lymphomas. Mycosis fungoides(MF) starts in the skin and tends to remain only in the skin. MF can appear as patches, plaques or tumours. MF often occurs in areas of the skin that are protected from the sun by clothing.

The sulfate salt of a vinca alkaloid extracted from Vinca rosea L.(Apocynaceae), C46H58N4O9. H2SO4; used as an antineoplastic in the palliative treatment of lymphomas, including generalized Hodgkins’ disease, lymphosarcoma, reticulum-cell sarcoma, and advanced mycosis fungoides, and of neuroblastoma, Letterer-Siwe disease, choriocarcinoma resistant to other agents, breast carcinoma resistant to other agents, and embryonal carcinoma of the testis. Administered intraveinously. Major side effects are nausea, vomiting, and bone marrow depression.

A treatment involving exposing patients to ultraviolet radiation after they have taken methoxsalen (8-MOP).

Recently there has been a rapid development in the world-wide use of oral and topical methoxsalen (8-methoxypsoralen; 8-MOP) with long wave ultraviolet light (UVA) for treating patients with psoriasis. This form of photochemotherapy is known as PUVA treatment, and is effective in clearing and maintaining a remission of psoriasis in most patients treated.

OBS

The light activates the drug, which then binds to DNA and causes cell death. Since the effects are confines to the skin, this is a treatment for mycosis fungoides as well as other skin disorders. A possible side effect of this treatment is the development of secondary skin cancers.

The leukemic variant of mycosis fungoides, [consisting] of generalized exfoliative dermatitis with edema, redness, and thickening of the skin associated with ectropion, leonine facies, keratoderma of the palms and soles, hepatosplenomegaly, and lymphadenopathy as well as with large numbers of atypical T lymphocytes in the circulation. The latter, so-called Sézary cells, represent T cells with highly convoluted nuclei identical to the cells infiltrating the skin in mycosis fungoides.
