... the concept of damage control... sees surgery as part of the process of resuscitation rather than an end in itself. In a damage control laparotomy for a critically wounded and exsanguinating patient the surgeon focuses on achieving haemostasis and preventing uncontrolled spillage of intestinal contents and urine, the laparotomy is abbreviated and the surgeon expends his or her efforts restoring the patient's... physiology in the surgical intensive care unit. Delayed gastrointestinal reconstruction can then be achieved following full haemodynamic resuscitation and restoration of coagulation variables to normal.

damage control surgery: Some authors use the term "damage control surgery" as a synonym of "abbreviated laparotomy" or "damage control laparotomy" to refer to a laparotomy for rapid control of hemorrhage and contamination. However, for several authors, "damage control surgery" refers to a three-stage process that includes an initial abbreviated laparotomy, followed by systemic management in an intensive care unit and a definitive repair surgery.

A test done during [a] physical examination to determine whether a patient with low back pain has an underlying herniated disk, often located at L5 (fifth lumbar spinal nerve).

CONT

When performing the straight leg raise test the patient is positioned supine in the absence of pillows. The clinician lifts the patient's symptomatic leg by the posterior ankle while keeping the knee in a fully extended position. The clinician continues to lift the patient's leg by flexing at the hip until pain is illicited or end range is reached. Neurologic pain which is reproduced in the leg and low back between 30-70 degrees of hip flexion is a positive result of lumbar disc herniation at the L4-S1 nerve roots. In order to make this test more specific, the ankle can be dorsiflexed and the cervical spine flexed. This increases the stretching of the nerve root and dura.

OBS

The patient is in the supine position with the shoulder closest to the x-ray tube abducted. The central ray is centered at the sternoclavicular joint and directed along the axis of the clavicle. On the normal Heinig projection the medial end of the clavicle closest to the film is projected through the manubrium, and the medial end of the clavicle closest to the tube is in direct alignment with the top of the manubrium. In the presence of a dislocation the medial end of the affected clavicle is then displaced either anterior or posterior to the manubrium.

ln addition to a tender point count, dolorimetry testing was done. A dolorimeter is a pressure algometer. It is a spring-loaded gauge attached to a rod with a soft rubber stopper at one end. The other end of the rod is attached to the spring. The patient undergoes testing by having certain anatomic areas placed in contact with the rubber stopper. The shaft of the instrument is then advanced slowly(about one kg. per second) by the examiner, and progressively greater pressure is applied until the patient complains of pain. At that point a numerical value is assigned to the area tested.
