The anterior drawer test is performed with the knee flexed at 90°. The examiner sits on the patient's foot and grasps behind the proximal tibia with the thumbs palpating the tibial plateau and index fingers palpating the tendons of the hamstring muscle group medially and laterally. The anterior tibial force is then applied by the examiner.... a positive test for a torn ACL [anterior cruciate ligament] is indicated by greater anterior tibial displacement on the affected side when compared to the unaffected side.

Blount's disease occurs in children with no apparent abnormality at birth, having a typical history of "genu varum" worsening with gait before the age of 2 years. The less frequent juvenile onset may occur between 4 and 10 years and the adolescent form over 11 years.... Blount's disease is believed to result from abnormal compression of the medial aspect of the proximal tibial physes, causing retardation of growth in that area or increased growth laterally of the proximal tibia or fibula... In juvenile Blount's disease, the etiology is less clear and may relate more to malalignment, leading to the characteristic changes visible on radiographs.

Again, a subluxation sign can be observed at about 30° flexion if anterior cruciate insufficiency is present.

Medial tibial stress syndrome(MTSS) is one of the most common leg injuries in athletes and soldiers. The incidence of MTSS is reported as being between 4% and 35% in military personnel and athletes. The name given to this condition refers to pain on the posteromedial tibial border during exercise, with pain on palpation of the tibia over a length of at least 5 cm. Histological studies fail to provide evidence that MTSS is caused by periostitis as a result of traction. It is caused by bony resorption that outpaces bone formation of the tibial cortex. Evidence for this overloaded adaptation of the cortex is found in several studies describing MTSS findings on bone scan, magnetic resonance imaging(MRI), high-resolution computed tomography(CT) scan and dual energy x-ray absorptiometry.

That the knee can attain any degree of stability in extension, and indeed is not even more vulnerable, is due to the all-important quadriceps and to the complex action which differentiates the joint from a simple hinge-namely, the screw-home movement. This is the mechanism whereby, at the completion of extension, the tibia rotates laterally by the indirect action of the quadriceps, guided in its course by the anterior cruciate ligament and the form of the articular surfaces. The screw-home movement is an important feature of the knee-joint mechanism not only because it determines the stability of the joint in extension but because disorders of its complex action are responsible for most of the common internal derangements of the knee.

We performed five transfers of the ipsilateral fibula in the treatment of congenital pseudarthrosis of the tibia. In all cases, conventional treatment by intramedullary-rod and autogenous-iliac bone grafting had failed at least once. We hoped to avoid the use of contralateralfibular grafting or other methods that have not yet been proven effective. The ipsilateral fibula was transferred, and its vascular pedicle left intact and secured to both fragments of the tibial pseudarthrosis by cerclage wires. All five transfers have united and hypertrophied with growth. We believe this salvage method is superior to others not only because of the successful results, but also because the intact limb is left undisturbed.
