The lateral pivot shift test of McIntosh. The examiner lifts the foot of the extended, internally rotated leg with one hand, forces the knee into valgus with the other hand by pushing on the side of the calf, and slowly flexes the knee while maintaining the valgus stress. In the jerk test of Hughston, the examiner flexes the knee to about 60°, grasps and internally rotates the distal tibia, and with his other hand applies a valgus stress to the knee. A test for ligamentous instability of the knee performed by stabilizing the femur while placing a valgus stress on the knee by abducting the leg. A moderate joint line separation with the knee fully extended indicates a tear of the medial collateral ligament and the posterior capsule.
